This application is now integrated into OHID, which gives you a single and secure account to log into State of Ohio services and applications.

• Access to Ohio Public Health Reporting Enrollment (OPHR) requires:
  • An OHID account, which can be created here.
  • Adding the OPHR App to "My Apps" from the OHID App Store.
    • Search for "OPHR" in the App Store.
      
    • When you have an OHID account and OPHR is in My Apps, you can click on that icon to access the application.
• If you already have an OPHR account, please try to use an OHID user account tied to the email address you used to create the OPHR account, so that we can identify your employer in the OPHR.
• If you are unable to access the application using your OHID account, please notify us by completing this form or calling 1-866-349-0002 during business hours.
