Ohio Public Health Reporting Enrollment (OPHR) is now integrated with OHID, a single and secure account to log into State of Ohio services and applications.

We need to know your OHID User ID so that we can link it to an OPHR account associated with the health care provider that you work for.

• Access to Ohio Public Health Reporting Enrollment (OPHR) requires:
  1. An OHID account, which can be created here.
  2. Adding the OPHR App to "My Apps" from the OHID App Store.
     • Search for "OPHR" in the App Store.
       
     • When you have an OHID account and OPHR is in My Apps, you can click on that icon to access the application.
     • If you are unable to access the application using your OHID account, please notify us by completing this form or calling 1-866-349-0002 during business hours.
